Ozempic and menopause: what GLP-1 drugs can and cannot do in midlife
GLP-1 drugs such as Ozempic, Wegovy and Zepbound appear to work for weight loss in menopause much as they do in other adults. Early studies hint that hormone therapy may add a little to the effect, but this research is small, preliminary and not proof.
Get urgent help for severe stomach pain that spreads to the back, pain under the right ribs, yellow skin or eyes, repeated vomiting, signs of dehydration, or face or throat swelling on a GLP-1 drug. Tell your doctor about shakiness or sweating if you also take diabetes medicines, worse hot flashes, or a possible pregnancy.
What GLP-1 drugs are
GLP-1 drugs are prescription medicines for type 2 diabetes and weight management. Semaglutide is sold as Ozempic and Wegovy. Tirzepatide is sold as Mounjaro and Zepbound. Mounjaro is the brand used for weight loss in the UK.
No large Wegovy trial was designed just for menopause, but women in perimenopause and menopause took part in the main trials. A 2026 review found weight loss was similar regardless of age.3
These drugs do not treat hot flashes or night sweats. There is no evidence that they ease these symptoms.3 See hot flashes and hormone therapy.
Does HRT help GLP-1 drugs work?
In 2025, Mayo Clinic researchers reported on 120 postmenopausal women taking tirzepatide. The 40 also using hormone therapy lost about 17% of body weight, compared with 14% for the other 80.1
This was a small study presented at a conference. It had not been peer reviewed or published in a journal, so the results are preliminary.1 The researchers called for larger studies.
A larger Mayo Clinic analysis of 2,379 women was presented at The Menopause Society meeting in October 2026. Women on hormone therapy lost slightly more weight, about 11.6% versus 9.7% at 15 months, with semaglutide.2 The tirzepatide results were not statistically clear.2
Both studies were observational. Women who use HRT may differ in health habits and access to care, so the studies cannot show that HRT causes extra weight loss. Hormone therapy is not a treatment for weight.2
Muscle and bone loss
Weight loss from any cause includes some muscle and bone. Muscle and bone already decline faster around menopause.3
In a small three-month study of 40 adults, lean mass made up nearly half of the weight lost on semaglutide. Older age and female sex were linked to more lean mass loss.4 The study was small and short, and lower protein intake was linked to more loss.4
Enough protein, strength training, and adequate calcium and vitamin D are commonly advised to help protect muscle and bone.3 See exercise and strength training and bone loss.
Side effects and safety
The most common side effects on the US Wegovy label include nausea, diarrhea, vomiting, constipation, stomach pain, headache, tiredness and hair loss.5
Label warnings include pancreatitis, gallbladder disease, kidney injury from dehydration and serious stomach problems. Semaglutide caused thyroid tumors in rodents. It is not used in people with a personal or family history of medullary thyroid cancer or MEN 2.5
The drugs slow stomach emptying, which may change how other pills are absorbed.5 One case report described worse hot flashes in a woman taking oral HRT, which a doctor may manage by changing the form of HRT.3
Pregnancy is still possible in perimenopause, and Wegovy may harm a developing baby.3,5 The drug stays in the body for weeks, so pregnancy plans need discussing with a doctor well ahead.5
Tirzepatide (Mounjaro, Zepbound) may make birth control pills less effective, mainly in the first weeks and after each dose increase. Its maker advises a non-pill or backup method at those times, so check with your doctor.7
A prescription medicine
GLP-1 drugs are prescription medicines in the US, UK, EU and most other countries. A doctor needs to check whether they suit you, watch for side effects and review other medicines.
Be wary of copies. The FDA warns that compounded versions are not reviewed for safety or quality, and has had reports of dosing errors, some needing hospital care.6 It also warns about counterfeit pens, illegal online sellers and products labeled "for research use only", which may contain the wrong amount or the wrong ingredient.6
Most people regain some weight after stopping.3 Decisions about starting, changing or stopping these drugs, or HRT, should be made with a doctor. For other approaches, see weight gain in menopause.
What it is used for: evidence rated
| Option | Evidence | What the research shows |
|---|---|---|
| Weight loss in midlife women | Licensed for weight management on prescription; large trials included women in and after menopause, with similar results across ages.3,5 | |
| Extra weight loss when combined with HRT | Only small or observational conference studies so far, not yet peer reviewed in full.1,2 | |
| Hot flashes and night sweats | No evidence that GLP-1 drugs ease hot flashes or sweating.3 | |
| Protecting muscle and bone | GLP-1 drugs do not protect muscle or bone; weight loss may reduce both, so strength training and diet matter.3,4 |
